_*HOURS:*_ Days, 8-5 or 9-6 (rotating)







Occasional travel to other sites to meet the needs of the clinic may be required.





_*Position Summary*_



Responsible for providing exceptional customer service while participating in the discharge planning process. Facilitates, and coordinates delivery of agency services by ensuring accurate demographic information and appropriate physician orders. Assumes responsibility and accountability for facilitating, communicating, and collaborating with both the healthcare team, and the patient/family to ensure an outstanding customer experience. Promotes the optimal health, well-being and safety of the patient through use of the nursing process and in accordance with patient care standards, guidelines and the State Nurse Practice Act.











_*Minimum Education Requirements*_



Graduate of an accredited Licensed Practical Nursing program











_*Minimum Experience Requirements*_



Two years of recent LPN experience One year of home care, hospice or home infusion experience preferred Previous experience in a discharge planning, preferred Requires knowledge in nursing, evidence-based practice methods, and person-centered care concepts











_*License/Certifications*_









* Current state LPN license for applicable state of employment or current LPN license from a participating state in National Compact Licensure (Colorado only)



* Current Cardiopulmonary Resuscitation (CPR) required; Current Basic Life Support (BLS) certification from American Heart Association, preferred LPN IV certification preferred



* Valid driver's license

* Communicates needs to the team as part of care plan development process; assists in implementing prior to discharge of the patient. Assists in coordinating services for patients as needed to other agencies and health care providers as appropriate.*



* Document ongoing status, interventions, patient response and outcomes in accurate, timely manner.*



* Professional and pleasant when answering calls. Accurately transfers calls in a timely and confidential manner.*



* Demonstrates the appropriate greeting when answering the telephone identifying self to and business to patients and families.*



* Recognize the patient or designee as a full partner in providing compassionate and coordinated care based on respect for the patient’s preferences, values, and needs.*



* Demonstrates competent thinking skills in order to handle multiple incoming calls and fluctuating volumes.*



* Documents all calls according to policy and procedure.*



* Promote, advocate and collaborate to protect the health, safety and rights of each patient.*



* Strive to understand what is most important to the individual and actively engage them in all aspects of care. This care will be delivered with respect for cultural differences and the diversity of human experience.*



* Maintain thorough and confidential patient charts and records.*
