*_Position Summary_*

The RN Navigator serves as the primary point of contact and is responsible for coordinating the care and services of the identified patient populations across the continuum of care. The incumbent promotes effective resource utilization and assumes a leadership role with the multidisciplinary team to identify and eliminate barriers to care in order to achieve an optimal clinical outcome for the patient and their caregivers.

*_Education Requirements_*

* Bachelor of Science in Nursing (BSN) required

* Master of Science in Nursing (MSN) preferred

*_Work Experience Requirements_*

* Three years of experience required, with one year in specialty

* Strong psychosocial assessment and clinical skills


* Current Colorado RN license

* Current BLS certification from American Heart Association required.

* Certification required within specialty, preferred

* ACLS and PALS required (Perioperative only)

* ONC required within 6 months of hire


* Demonstrates the knowledge, skill and coordination to provide nursing care and guidance to the patient from first contact and throughout the continuum of care.*

* Guide patients on treatment plans, next steps in appointments (i.e. surgical prep, imaging, labs, tests, which provider to see next, etc.)*

* Assist with scheduling follow-up appointments and provide information and reminders to patients and physicians offices.*

* Ensures an extraordinary customer experience. *

* Track outcomes of referrals and treatments. *

* Maintain detailed records to measure effectiveness of program and suggest ways to improve throughout the process.*

* Provides psychosocial support that may include facilitation of support groups.*

* Develop patient education programs and tools.*

* Provides formal and informal education to other members of the health care team to achieve desired outcomes.*

* Coordinates and participates in community education programs, health fairs and local events.*

* Assists multidisciplinary team in implementing clinical plan of care.*

* Educates the patient and family on care specific guidelines and treatment options to enable patient-led treatment decisions. Educates and facilitates patient on introduction to program clinical trial information, resource referrals, and counseling.*

* Advocates for patients and families in preventing/resolving clinical issues; develops supportive unit practices and guidelines.*

* Develops strong relationships with physicians, their office staff, community resources and healthcare providers to assure effective communication in patient care. Serves as a liaison between clinical specialists, primary care physicians and patients.*

* Serves on accreditation committees for the specialty of practice. *

*_Physical Requirements_*

Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10 lbs. constantly.
