_*Position Summary*_



Provides psychosocial assessments, support and counseling to oncology patients per the cancer center protocol from pretreatment through survivorship. Works closely with the cancer center RN Navigators to coordinate care. Provides support to physicians and hospital personnel to assist in providing optimal psychosocial support of patients and families. Attends tumor board meetings, multidisciplinary clinics, and other meetings as required for multidisciplinary patient care. Assumes personal accountability for organizational mission and core values.



_*Minimum Education Requirements*_









* Master’s degree in Social Work (MSW)









_*Minimum Experience Requirements*_









* Three years recent experience in social work, preferably in a healthcare setting



* Hospital Social Work experience strongly preferred



* Oncology Social Work experience preferred









_*License/Certifications*_









* LCSW strongly preferred









_*Position Duties (Essential Functions)*_







*Delivers High Quality Patient and Family Support*









* Assesses patient’s psychosocial, cultural and spiritual needs through observation, interview, review of records and clinical data. This includes assessment of family relationships, support system, mental health history, personality traits and coping skills. Evaluates patient and family response and intervenes in a timely manner to ensure optimal patient outcome. Conducts informal and formal family and nursing conferences with the RN Navigator to ensure most efficient coordination of health care and related services. Conducts individual or group therapy sessions as needed. Assesses patients’ progress and modifies or extends treatment program accordingly.









*Manages Plan of Care*









* Plans, organizes, directs and coordinates health care, community and related services as needed for patients being discharged from hospital to home in collaboration with case management. Utilizes resources to help patient resume life in the community or to learn to live with disability. Monitors ongoing services and plan, provide continuing contact with patient and family to coordinate ongoing care. Engages patient/family to foster commitment to comply with plan of care. Assists patient to learn to live independently, utilize community resources, vocational rehabilitation. Collaborates with interdisciplinary transplant and hospital team to develop and implement holistic, individualized plan of care.









*Performs Functions Specific to Transplant Social Worker Role*









* Serves as resource to nursing and medical personnel. Attends and participates in transplant team meetings and committee functions as required. Participates in educational programs and inservice meetings. Collaborates with and provides support to referral base (dialysis centers, community physicians). Performs other related duties as assigned or requested, which may include cross-training and/or other job functions as temporary work loads and volumes require.









*Maintains Complete and Accurate Patient Records*









* Thorough and confidential maintenance of patient charts and records. Documents all relevant aspects of psychosocial care, education and patient response and outcomes in an accurate and timely manner according to departmental standards.

* Serves as a patient advocate. Anticipates needs of patient, family, and staff. Effectively uses time, personnel, equipment and supplies to provide high quality, cost effective delivery of psychosocial and community services. Adapts to rapidly changing technologies, policies, regulations, workload and staffing through sound critical thinking, priority setting and flexibility. Role model for all staff demonstrating ethical/legal/professional standards. Maintains high skill level through ongoing education/in services, meeting attendance. Pursues/Maintains licensure/accreditation as Social Worker.

* Medium Work - exert/lift up to 50 lbs force occasionally, and/or up to 20 lbs frequently, and/or up to 10 lbs constantly
